Caeruloplasmin is a circulating copper binding protein and an acute phase protein which is increased in inflammatory responses or conditions. Deficiency of caeruloplasmin results in deposition of excess copper causing tissue damage.
Caeruloplasmin deficiency is characteristic of Wilson’s disease, which often presents as liver disease. It is also seen in the rare neurodegenerative disorder Menke’s disease (also known as Kinky Hair Syndrome). Caeruloplasmin should be measured as an initial screen for Wilson’s disease. If caeruloplasmin is low or Wilson’s disease is still suspected, 24 hour urine copper should be measured to further investigate. Note serum copper is not useful in the investigation of Wilson’s disease.
