Reference range dependent on target set by pharmacy clinics for each patient.
INR >5.0 (patients on warfarin): results will be telephoned within routine working hours only. Out of hours samples may be telephoned the next working day.
INR >8.0 (patients on warfarin): samples which specify Pharmacy as the requestor will be telephoned to the hospital pharmacy department within their routine working hours (09:00-17:00).
Outside of these times and for all GP samples the result should be telephoned to 111.

International Normalised Ratio (INR) measurement is used for monitoring Vitamin K antagonist anticoagulant therapy (primarliy Warfarin). The patients Prothrombin Time (PT) is divided by the laboratory Mean Normal Prothrombin Time (MNPT) and the resulting ratio is then raised to the power of the ISI, to convert results to a common WHO standard.
