Soluble Transferrin Receptor (STFR)

Cellular uptake of iron is controlled by expression of the transferrin receptor on cell membranes. In plasma, soluble transferrin receptor (STFR) forms a complex with transferrin. Levels of STFR are directly proportional to membrane-bound transferrin receptors.  

SFTR is measured alongside ferritin in the investigation of iron deficiency and a STFR-ferritin ratio is calculated: 

  • <1 iron deficiency unlikely 
  • >2 iron deficiency likely 
  • Ratio 1-2 AND raised SFTR – check haemoglobin possibly a short course of iron
